I created a simple workflow using SharePoint Designer to automate document approval.
Most of my documents don't need to be approved, so I added an AutoValidate column to my document libraries.

The workflow principle is the following :

If the document AutoValidate field value is true then
    Set the document status to Approved.
Else
    Send a e-mail notification to the approver.
   
I tested this workflow adding a small number of documents into my libraries and it seems to work
even if I don't understand why some documents are processed after 2 minutes and others after 1 hour.
But when I add large number of documents into my libraries (about 2000), I see that many documents are still not processed
one month later.

Any suggestion are appreciated.

Microsoft has confirmed that a single view within a document library should contain no more the 1500 records (Lawrence Liu speaking to NYC SharePoint User Group, Aug 2007). He said they are investigating the cause, acknowledging that it is a major concern.

I'm surprised that you are still able to function at all having that many documents visible in a single library. Try doing it a couple hundred documents at a time, moving the processed documents into an archive center and then processing the next batch. I think you might find you'll start getting better results.
